The Centers for Disease Control and Prevention (CDC) has released a funding opportunity announcement (FOA) for implementation of population-based strategies that address the greatest predictors of chronic disease, including tobacco use and exposure; poor nutrition; physical inactivity; and lack of access to chronic disease prevention, risk reduction, and management opportunities. Funding is intended for organizations who work through multi-sectoral community coalitions. CDC estimates that up to $150,000,000 may be made available to fund 30-40 awards of $100,000-$4,000,000 per year. Projects may last up to three years. Eligible applicants include government organizations or IRS-recognized nonprofit organizations. Coalitions who have worked with their members and other partners to complete a community health needs assessment in the last three years may wish to consider applying for this cooperative agreement.
