On Tuesday, just days before the end of the federal government’s fiscal year, the Department of Health and Human Services (HHS) announced that $103 million in Community Transformation Grants (CTGs) were awarded to 61 recipient organizations in 36 states. CTGs are part of the multi-billion dollar Prevention and Public Health Fund that was part of the ACA health care reform legislation and are focused on reducing the prevalence of chronic disease through primary prevention strategies at a community level.

Here is a copy of the HHS press release and a complete list of the awardees. NBCH coalition members are encouraged to reach out to awardees in their state or community to discuss partnership opportunities related to employer/business community participation in CTG related activities. Each grant recipient must demonstrate that they are actively engaging a coalition of different stakeholder groups, including the employer community.
